2011 Will be a year to celebrate for our Eye Care / Optometrist Practice in Fort Collins. I have had the honor of serving the Ft Collins Community since 1985. Over that time we have performed eye exams, fit contact lenses and eyeglasses, and treated eye diseases for thousands of residents in Northern Colorado. We now have soft contact lenses suitable for the Fort Collins humidity available in toric astigmatism formats. Next we do ask those one or two questions when you sit behind the phoropter, the mask like instrument in the picture above. You are free to request that we slow down or repeat the comparison of lenses at any time during your eye exam. I always want my patients to feel like they are free to ask for whatever they need to feel comfortable with the final prescription we arrive at. If you need more time than we have scheduled you can reschedule for a second or third eye examination absolutely free. All of those one’s and two’s may seem confusing but it is more like tuning in a radio station. We go back and forth repeatedly verifying you are giving consistent responses to the same questions. Here is a secret eye doctors are loathe to reveal-sometimes the two different views are identical!So don’t feel bad if you really can’t tell the difference. I add a few extras to my eye exams when needed. Most Optometrists and Ophthalmologists no longer have the skill to utilize retinoscopy, the hand instrument technique that was used in the past. I use it frequently when a patient is having trouble telling the differences during the eye exam. This is also totally independent of verbal responses-and I go a step further. I will utilize readings from the eye computer and retinoscope before and after putting in dilation drops. These drops eliminate most of the variability in focusing that alter eye prescription readings. Even when children or adults are completely non verbal our testing sequence usually yields an eye correction of 20/20. Rest easy, we can provide you with an eye exam you can’t fail!

New Drug For Macular Degeneration Setting Sales Records

The new drug treatment for macular degeneration Eyelea is racing past all projections for its sales since its release in Nov 2011. The first six weeks were announced to have around $25,000,000.00 worth of the drug sold for injection into the eye. That is about 500 people per day receiving injections of Eyelea for the last 6 weeks.

Is This A New Wonder Eye Treatment Or Really That Much Better?

The primary advantage for patients with Eyelea is the recommended treatment cycle is every two months versus the monthly injection for Avastin or Lucentis. No one is in a rush to have a shot into the eye so   that is a very strong selling point. The true reason for the unexpected surge in sales probably has little to do with the frequency of injections or how Eyelea is seen to perform. The most likely cause is a small number of cases of Avastin infections that occurred inside eyes after treatment in the second half of 2011. Compared to the cost of Lucentis and Eyelea Avastin is dirt cheap ($50 compared to $1700 TO $1800 per injection). Avastin does not come in single vials like Eyelea and Lucentis, and there is the potential for contamination when it is transferred to unit doses. Since one company (Genetech) manufactures both Avastin & Lucentis it is not expected that Avastin will be retested for on lable use in macular degeneration and marketed as a single vial drug. 

What is The Future For Eyelea?

Eyelea is expected to take over about 10% of the market for eye injection treatment of macular degeneration in 2012. As the shadow falls off of Avastin, I expect it to return to popularity due to the cost and higher oversight over repackaging. Genetech is working on a new device to allow delivery into a port without frequent injections, but that appears a way off in the future. Topical eye drops for macular degeneration are expected to enter the market within the next year. Eyelea probably won’t exceed expectations anywhere near the initial release results, but it should do well. Don’t feel like you need to rush out and change the type of treatment you are currently receiving. Eyelea probably won’t treat macular degeneration any better than existing drugs. On the other hand, if the thought of frequent shots in the eye are keeping you from seeking treatment ask about Eyelea, it just might save your sight.


Contact Lens Glaucoma Test

Can A Contact Lens Help Diagnose Glaucoma?

 

A company called Sensimed is improving the early diagnosis of glaucoma with a “wired” contact lens. One of the dilemmas in understanding early glaucoma detection is testing the eye pressure at the right time. Eye doctors have found eye pressure fluctuates throughout the day and a single reading may yield the pressure at a low point giving a false impression of the state of health of the eyes. That is why patients that are suspected of having glaucoma are often seen by their optometrist at different times of the day to retest the pressure. While this does help in getting a more accurate idea of the true eye pressure, it is not practical to return every few hours in the middle of the night for glaucoma testing, thus the need for something like the new Triggerfish Contact Lens.

Triggerfish Contact Lens To Allow 24 Hour Monitoring Of Eye Pressure

The Triggerfish contact lens system consists of the contact lens, an antenna, and a recording and transmission unit. The contact lens has strain gauges to detect changes in curvature at the edge of the cornea (the clear tissue covering the colored iris).  The changes in curvature correlate with changes in eye pressure. An antenna resembling a patch around the eye is attached for the 24 hour test. This antenna is tethered by a thin cable to a recording device worn on the patients belt. The antenna provides power to the contact lens by induction, a wireless method of transmitting electrical current.

Once the contact lens has been fit to the patients eye curvature, the device transmits data to the recorder for the next 24 hours. Eye doctors have found eye pressure tends to be highest at night, so a 24 hour cycle is important in detecting the highest pressure levels the patient is at risk for developing. After the 24 hours, the patient returns to the eye doctors office and the data is uploaded to a computer by Bluetooth.

Early Detection Of Eye Presuure Prevents Blindness

Early intervention in glaucoma can prevent blindness. Studies have found that some early damage to the optic nerve can be reversed by treatment. The Triggerfish contact lens is currently undergoing clinical trials for the U.S. but is already available in parts of Europe. This is potentially a sight saving test that has been needed for years.

Someday the contact lens you wear for vision correction may have dual functions as a health monitoring device. Beam me up!

Who has the distinction of having the worst customer service in the credit card industry. IMHO Chase wins that hands down! A company that does not care about their clientele reaches new lows on a daily basis. Go somewhere else.

In January 2011, JPMorgan Chase admitted that it wrongly overcharged several thousand military families for their mortgages, including active duty personnel in Afghanistan. The bank also admitted it improperly foreclosed on more than a dozen military families; both actions were in clear violation of the Servicemembers Civil Relief Act which automatically lowers mortgage rates to 6 percent, and bars foreclosure proceedings of active duty personnel. Beware of cutting down that Christmas tree! Every year flocks of people show up in eye doctors offices from Christmas Trees that have gone to the dark side. Christmas trees attack people for different reasons (lack of Christmas Spirit mostly) and in different ways. Be aware and be safe. Never get caught in a dark chimney alone with a Christmas tree. 

Tree Needles Can Damage Eyes When The Branches Are Untied

 

When you unbundle that new spruce tree in the living room make sure to have the kids stay a few feet back. Branches are released like a spring and needles have been known to scratch the cornea on the front of the eye and even puncture all the way through. 

Looking For The Last Christmas Present May Have You Looking For An Optometrist

Children reaching under the tree for that last package wedged way in the back may end up scratching the front of their eye from low lying branches. Either keep the presents towards the front or reach the ones in the back for them.

Boulder Colorado

it was developed by some bored computer programmers who were supposed to be developing Star Wars Technology. Someone reportedly spilled a Mt. Dew on a keyboard which of course shorted out launching three prototypes towards Iceland. The lead tech had reached for the Dew as it tipped but just missed it. Just short of launching Armageddon, the team was quite mad at him-to which he replied “I thought I had her”. 

A government observer commented on how mad everyone was in spite of the “Had Her” and, well the rest is history. Mad Had Her became Mad Hatter. Our survival is now celebrated as Mad Hatter Day.

Every top hat present will testify to the veracity of this story. If you don’t believe it, go on ahead-you will get to the top of it at some point in the future!

Can someone look into your eyes and know your mental state? A study of children with parents who were either bipolar or schizophrenic seems to indicate this may be true.

Special Eye Tests Measure Electrical Changes For Night Vision

The electroretinogram uses a special contact lens on the front of the eye that records the difference in electrical charges from the front and back of the eye. The b-wave measures alterations in this charge by the rod cells responsible for night vision. Eye doctors found this wave was significantly reduced in children having at least one parent effected by either bipolar disorder or schizophrenia.

Eye Doctors  May Check Your Mood And Your Eyes In The Future

Electrical testing of eye function is still used largely in teaching institutions.  This equipment has become much more compact and inexpensive and will most likely be seen in many eye doctor offices in the future. The first step in prevention is knowing who is most susceptible to a disease. Hopefully there will be ways to intervene at that early junction and prevent these disorders from ever manifesting any symptoms.

 

 

 

 

 

 

 

 

How Long Can You Use Eye Drops?

To Use or Not To Use (Opened Eye Medications)

Can you keep using those old eye drops and when should you throw them away? It is not the easiest question to answer but there are some guidelines.

Eye drops come in single use containers (small plastic vials with twist off caps) and in multiple dose bottles. The multidose bottles normally contain preservatives to prevent contamination. Frequently dropper tips touch the eyelids or countertops if you are not very careful. That point of contact can contaminate the bottle with bacteria. Preservatives are added to help overcome any inadvertent contamination. Preservative taken over the course of years can damage some of the surface cells on the eyes tissues and contribute to dry eye syndrome and red eyes. Preservative free single use eye drops have been developed to circumvent this problem, but they come with a hefty price tag.

Refrigerators And Eye Drops-Cool Or Not!

Most prescription eye drops are OK to use for 30 days after opening if they are kept refrigerated. Most bacteria are inhibited below 40 °F and above 140 °F. (The listeria bacteria that have been in the Colorado news of late are an exception since they thrive in cooler environments). Not only does refrigeration inhibit bacterial growth but it also helps maintain the stabilty of the drops and prevent them from degrading into other compounds. One study found the glaucoma eye drop Lumigan was much more stable than the well known xalatan drop at room temperatures. For people with glaucoma that spend frequent time traveling, eye doctors may prescribe an eyedrop like Lumigan when refrigeration is unlikely to be available. For about every 20 &deg:F rise in temperature drugs double in their instability. One thing to keep in mind is that refrigeration does not stop bacterial growth, it only slow it down. Prescription eye drops can become contaminated from bacteria inside of foods and refrigerators so it may not be the best choice for drops that are used only on a short term basis- and you should keep that thing clean anyway!

Who Knows What Really Lurks In Older Eye Medications-The Government And They Are Only Guessing!

Most prescription eye drops have a range of concentrations that are effective in killing bacteria. The potency of antibiotics is usually based on complex calculations that may not be accurate. A few drugs like tetracycline actually degrade to products that increase the resistance of bacteria to treatment. Determining shelf life is no easy task so the FDA tends to err a little on the conservative side. Eye institutions like Mooresfield Hospital in the UK frequently prepare unpreserved multiuse drops for special, severe eye infections. They have tested the sterility of these drops over the years and found that non preserved drops are typically safe to use for a week when refrigerated. Most studies indicate that multiuse preserved eye drops are good for 1 month after opening when refrigerated. 

The rule of thumb is a week for non preserved multidose drops that are kept refrigerated and 30 days for preserved multidose drops that are kept refrigerated. A study of cycloporine, the same eye medicine found in restasis for dry eye treatment, also indicated 1 week when refrigerated. The formulation was slightly different, and restasis is approved as a single time use per vial. Some eye doctors allow their patients to use the same vial for 24-48 hours. 

When you have opened eye drop prescriptions it is best to discard them after 30 days. As always, you should ask your prescribing optometrist first before using any eye medications in a manner that has not been through a formal approval and testing process. And clean that refrigerator!

Are Eye Supplements Safe?

Almost everyone takes some form of supplements these days. Many things fall under the definition of supplements including herbs, vitamins, minerals, enzymes, and a host of other categories. These supplements categories were laid out in the Dietary Supplement Health and Education Act (DSHEA) by congress in 1994. They must be labeled as dietary supplements and ingredients, and not as food or medicine. This act changed supplements from being regulated as food items to a new class. So are they safe?

The Manufacturing Company Is Now Responsible For Eye Supplement Safety Instead Of The FDA

It is the manufactures responsibility to make sure existing supplements are safe and that any assertions made are true and not misleading. There are no requirements for testing or FDA approval for exsiting supplements. While herbal manufacturing companies have liability for any problems that arise from the use of their products, that is something that occurs after the fact. New supplements are required to undergo a review process. The issue underlying this procedure is the lack of a defining list of existing supplements. Since the manufacturer is responsible for determining if the dietary ingredient is new or if it was previously found in food, there is realistically no guarantee of a review for new supplements. The Good Manufacturing Practices Regulations were passed in 2007 to assure the purity, quality, and content of supplements. While it provides a process for record keeping and inspections the mainstay of quality control is dependent on the reports of adverse events or complications arising from the use of a supplement.

You can check for adverse event reports at Dietary Supplements – Adverse Event Reporting. Being informed about the products you are taking and the reputation of the company is probably the best assurance you can find about their safety and usefulness. Your eye doctor can also be a resource for finding the best brands available.
