Next we do ask those one or two questions when you sit behind the phoropter, the mask like instrument in the picture above. You are free to request that we slow down or repeat the comparison of lenses at any time during your eye exam. I always want my patients to feel like they are free to ask for whatever they need to feel comfortable with the final prescription we arrive at. If you need more time than we have scheduled you can reschedule for a second or third eye examination absolutely free. All of those one’s and two’s may seem confusing but it is more like tuning in a radio station. We go back and forth repeatedly verifying you are giving consistent responses to the same questions. Here is a secret eye doctors are loathe to reveal-sometimes the two different views are identical!So don’t feel bad if you really can’t tell the difference. I add a few extras to my eye exams when needed. Most Optometrists and Ophthalmologists no longer have the skill to utilize retinoscopy, the hand instrument technique that was used in the past. I use it frequently when a patient is having trouble telling the differences during the eye exam. This is also totally independent of verbal responses-and I go a step further. I will utilize readings from the eye computer and retinoscope before and after putting in dilation drops. These drops eliminate most of the variability in focusing that alter eye prescription readings. Prism Eyeglasses-Eye Doctors Treatment For Double Vision

Optometrists in Fort Collins Usually Prescribe Prism Eyeglasses For Double Vision

Has your eye doctor prescribed prism eyeglasses for you or your child in a new eyeglass prescription? It could be due to crossed eyes, lazy eye, or some diseases of the eyes or body.

Prisms are thin pieces of the optical material that is used in prescription eyeglasses. You may remember them from high school physics. They have a base that is thicker and an apex that is thinner. Due to the light bending properties of the lens material, the thicker lens base slows light down as it passes through. Since the prism is thicker at the bottom base it slows light down longer than the top apex, and light changes direction down towards the base as it exits the prism. If the lens were equally thick throughout, it would slow the light down but the direction would remain unchanged. The amount of direction change is determined by the index of refraction of the material compared to the index of refraction of air. Lenses with higher indexes of refraction allow optometrists to make the ultra thin lenses that have drastically improved eye comfort and cosmetic appearances in recent years. The light bending capacity of a lens material is referred to as the index of refraction. The value in space in a vacuum is 1.0, essentially the same as in the earth’s atmosphere which is 1.0003.  Eye glass lenses now have indexes of refraction from 1.5 to 1.7, with the higher numbers being the thinnest.

Eyes that cross can turn in or out, and also one eye can turn up or down. Strabismus is the technical term for a turned eye. Eyes that turn out are referred to as exotropic while eyes that turn in are called esotropic. An eye that turns up is hypotropic and one that turns down is hypotropic. These are all considered as cases of strabismus, or eyes that actually do cross. Often they are mistakenly called “Lazy Eyes.”  A lazy eye is a different condition but it may result in a crossed eye. A combination of eyes turning laterally and vertically is common. When this results in double vision, it can be completely disruptive to mobility and lifestyle of the affected person. If the eye turns are present at birth, there may be no double vision present. The brain has the capacity to suppress or turn off the area of vision that results in the eyes perceiving double at early ages. When a person sees double, prisms, eye exercises known as vision therapy, and surgery are the three options eye doctors have to try and restore normal visual functioning. Prisms do not appear as a strange looking triangle in the lens. They usually show up as thicker and thinner edges on the eyeglass lenses. Normally they are ground into the shape of the lenses, but because of optical properties, some lenses can have the optical center repositioned to induce prism. There can be an adjustment to prism added to a prescription while the brain relearns how to interpret the eyes seeing single. When diseases such as strokes and diabetes cause double vision there can be some fluctuation over time, and frequent eyeglass prescriptions changes to adjust the amount of prism may be required. Temporary press on prisms known as Fresnel lenses may be used in some cases when change is expected but their optical quality tends to be very poor.

Your eye  doctor may prescribe prism in your eyeglasses for other reasons

Some people will only have a tendency for an eye to turn, and while it may not actually turn it will result in eyestrain, fatigue with reading, headaches from using your eyes, and other symptoms. These are referred to as heterophorias instead of strabismus. Instead of exotropia your optometrist would call it exophoria. Any term ending in phoria means a tendency to turn while tropia indicates an eye that is turned. To complicate matters, sometimes eyes will cross at one distance and not another, or when viewing in only certain fields of gaze. In cases of traumatic head injuries yoked prisms may be used to help retrain a disrupted visual system by altering spatial perception.

Some serious health problems can cause double vision, and any new onset or increasing condition of double vision should be thoroughly examined by your optometrist

When double vision occurs after the age of fifty, common causes are thyroid conditions, high blood pressure and diabetes. Try to notice when it occurs and what makes it worse to help your optometrist in treating your eye condition with the best methods possible. In the case of a brain tumor inducing double vision, a trip to the eye doctor could save your life.

Many eye doctors office claim they provide specialty contact lenses or are contact lens experts. We know the frustration you feel when it turns out they are really only interested in simple cases they can shuffle in and out the door in a hurry.

Specialty Contact Lenses We Provide Include:

  • Kerataconus-This is an eye disease where the clear corneal tissue on the front of the eye thins develops a very steep curvature with an irregular curvature. A cone shaped protrusion forms on the cornea. Many different special contact lens designs are used to compensate for this shape including soft contact lenses, gas permeable lenses such as the Rose-K designs, hybrid lenses with soft lens material on the outer edges and RGP rigid gas permeable materials on the inside, piggyback lenses that combine a gas permeable lens on top of a soft lens, and scleral lenses that extend over the white tissues on the eye. Kerataconus contact lens fitting is still very much an art form with no exact science.
  • Cosmetic Lenses For Eye Damaged by Trauma: The colored part of the eye, the iris may be mishaped from birth or by trauma leaving an oddly shaped pupil. Specialty Colored Contact Lenses can be used to restore a normal pupil appearance and cut down on glare and discomfort from a malformed pupil.
  • CRT or Orthokeratology Contact Lenses: Gas permeable lenses can be used in some cases to reshape the corneal tissue on the front of the eye and reduce nearsightedness (myopia). This is often done by wearing a lens overnight so the eye is prescription free during the day.
  • Large Amounts of Astigmatism: New advances in contact lens technology allow for large degrees of astigmatism to be corrected by contact lenses. Usually the comfort of soft contact lenses can be  used in a disposable contact lens for health and comfort.
  • Contact Lenses After LASIK Laser Eye Surgery: LASIK doesn’t always work and the eyes may be left with a very irregular surface. They may also be left with residual corrections and change with time. These issues can be solved with specialty contact lenses, when glasses may not provide a good correction due to irregularities in the astigmatism.
  • Color Vision Defects: While contact lenses cannot restore normal color vision they can make it easier for some patients to discern differences.
  • Nystagmus or Eye Tremors: In some cases, rigid contact lenses can supply feedback to the brain about these uncontrolled eye movements and help dampen the eye movements to some degree.
  • Contact Lenses For 40+ Year Old Eyes Needing Bifocals: If your arms are getting too short too read, don’t despair-bifocal and mutlifocal lenses are available in many different designs in soft and gas permeable contact lenses. Expect to spend some time being fit.

How Long Do Eye Dilation Drops Last?

Patients frequently ask me how long their pupils will remain dilated after the eye exam. Eye Doctors use different drops,different strengths, and different dosages when dilating your pupils. If you are having light flashes or symptoms that could indicated retina problems two different drops may be used in higher concentration and several doses. A routine exam may only require less dilation and milder drops.

Eye color effects the duration of dilation. Since brown pigment in the iris is essentially the only eye pigment, blue eyes lack significant color and gain their blue shade from light reflecting in the iris. Pigment binds the dilating drops and require higher doses but blue eyes react faster and more to dilation drops.

Different drops are often used for children’s eye exams and occasionally for adults that have inflammation in the eye from contact lens injuries, eye trauma, or autoimmune diseases and they may keep eye dilated for several days.

Finally, there are individuals with idiosyncratic reactions to the prescription eyedrops used to dilate your eyes. Basically, that means the optometrist is an idiot and has no idea why one out of hundreds of eyes are extremely sensitive to dilation drops and stay dilated for 24 hours or more. (The meaning of idiosyncratic is a well kept secret by all types of Doctors so please don’t reveal me as the source of explanation). If you have this type of reaction you can try and request a little known secret we use-simply diluting the drop!

The final answer is your eyes will usually stay dilated for several hours but it could be up to several days. Good luck and remember, after the first time you will probably have a fairly good idea of what to expect in the future.

Contact Lens Solution Allergies

We still see allergies to the preservatives in contact lens solutions in our Fort Collins Contact Lens Clinic but at a much lower frequency than in the past.

Thimerosal, (sodium ethylmercurithiosalicylate)  is an older contact lens solution preservative whose composition is about half mercury. It as commonly used in very low doses in contact lens disinfecting solutions is an excellent preservative but causes a reasonably high number of allergic reactions. It has been used in childhood vaccines and a subject of controversy over the years as a possible cause of autism. There are no significant studies to confirm a relationship,  but similar to contact lens solutions, it is being phased out. You will often see vaccines that are touted as mercury free.

Chlorhexidine is a chemical antiseptic that is very effective at killing bacteria. It was also widely used in the past in contact lens solutions prescribed by eye doctors but frequently caused red eyes, vague forms of  eye irritation, and sometimes significant conjunctivitis and  discomfort. Like Thimerosal, it has largely been phased out of contact lens care and we never reccomend it in our Ft Collins Eye Practice. It is still widely used in the dental field to treat gum disease and as a disinfectant.

Benzalkonium chloride was used in the past and is still used today is some nasal sprays, cleaning solutions, and various other hygienic products. It is a good disinfectant but at high enough levels may not only serve as an allergen but cause serious damage to the corneal tissue the contact lens rests on.

The next generation of sensitive eyes contact lens solutions proved to have about the same rate of allergic reactions, just for different people. Switching often cured the problem for those allergic to former solutions but created problems for new users.

Many generations of solutions have since come to pass. Today the large molecule preservatives are favored by optometrists since they fail to build up in the contact lens matrix, exceeding the pore size of the lenses.

The problem today rests in generic contact lens solutions, often colored and labeled to look like the brand solutions. Sometimes they are OK, at other times they are using these older toxic preservatives that should have been off the market a long time ago. To compound the confusion, the formulations may change in the middle of the shelf for the same generic brand, even though the box appears identical.

The best bet today is to stick with a brand name solution your eye doctor reccomends or use one of the hydrogen peroxide based systems. The drawback to hydrogen peroxide has always been it is too easy to make a mistake, and hydrogen peroxide is very uncomfortable when placed in your eye without neutralization!

Spend a few extra minutes during your next visit to the optometrist and ask their advice on what is the best contact lens care regimin avaialbe to keep your eyes healthy and seeing well.

Common Causes For Childrens Headaches From Eye And Vision Disorders

Headaches come from many different sources and people often bring their child to see the eye doctor first to rule out vision problems as a cause. Headaches are the second most common reason parent bring their children in to our Fort Collins Optometrist,  Practice Usually it is not a vision related problem, but there are cases that have a direct correlation with the eyes. If headaches are related to eye problems, most of the time your optometrist will uncover a specific visual task the headaches seem to center around. It could be reading, computer use, video games or time in the sun.

Eyeglass frames can cause pressure behind the ears and on the side of the head if not properly adjusted. Frames that are tilted can alter the effective lens prescription and result in eye strain. Nickel is a common component in many metal frames (and in parts of plastic frames). Nickel allergies are fairly common and can cause discomfort, itching and possible mild headaches.

Your child’s eyeglasses prescription may cause headaches under certain conditions. Large uncorrected amounts of farsightedness are probably the most common cause in children. With farsightedness, they may have the focusing capacity to pass the eye doctors chart test with flying colors while not wearing prescription lenses. In doing so, they may be close to using all of their focusing reserve capacity. This would be like spending all day walking around carrying close to the maximum amount of weight you can hold. Because the focusing muscle is considered smooth muscle and does not fatigue like the striated muscles you use for your arms or legs, there is some disagreement on this point. It is really an academic point since the headaches do commonly occur, possibly due to variations in focusing and resultant clarity, constriction of the colored iris tissue, or other unknown factors. How much uncorrected farsightedness is required to cause headaches? We know higher amounts are more significant as age increases but with lower amounts it is not as clear what levels create eyestrain and headaches. Sometimes the only way to know is to fill an eyeglasses prescription and see if the headaches resolve.

Focusing problems in general are very hard to diagnose with precision. Eyes may over focus, under focus, have variable focusing, and on rare occasions have focusing spasms. Some prescription medications can cause focusing problems, dry eyes and sensitivity to light increasing the likelihood of headaches.

Uncorrected nearsightedness can cause a child to squint to see the blackboard and result in headaches. Usually they will be complaining about blurry vision, unlike with farsightedness. Nearsightedness has normally been corrected by an eye doctor due to blurry vision before there are complaints about headaches.

Astigmatism is a condition where the eye has two different curvatures, shaped more like half of a tennis ball squeezed on top and bottom. While astigmatism does blur vision, children frequently notice more eyestrain than blurriness. The eye is focusing for one curve then the other trying to find the clearest focus point. Moderate to high levels of astigmatism can cause headaches but usually the child has complaints that sound more like eyestrain and may be squinting to try and clear things up.

Convergence insufficiency is one of the leading causes of headaches related to vision in children. In our optometry practice in Fort Collins, we find it is one of the most common undiagnosed eye conditions. When your child reads or works up close the eyes have to perform two functions. First they have to adjust the focus for the correct distance. Secondly they must turn in both eyes (converge) to point in the exact direction of what they are looking at. Most reading is done at about 14 to 16 inches away from the face in children. Kids should be able to keep their eyes pointing at an object at least until it is within 4 to 6 inches from their nose. If they can’t, they lack enough reserve capacity to keep their eyes pointed at the object and they will have eyestrain and headaches. The severity of the problem is related to how well their brain functions in partially shutting down the image from one eye. If they lack this capacity the headaches can be severe with near work, resulting in headaches, eyestrain, blurred vision, and failing grades in school.

Muscle imbalances are similar to convergence insufficiency. Due to eye muscles or tendons that are slightly misshaped or inserted slightly abnormally, their eyes may have a tendency to turn in, out, up, or down. Constantly struggling to keep images from doubling can cause frequent headaches if the brain is not good at shutting off one of the images. Fixation disparities are very tiny eye alignment errors that can have a similar result but can only be diagnosed with appropriate testing.

Migraines are a very common cause of headaches and take many different forms. Usually there is a family history of migraines already known. Children that develop migraines at an older age often are carsick at younger ages. These headaches tend to be on one side of the head and may be associated with nausea and light sensitivity. Migraines need to be thoroughly tested and diagnosed in conjunction with your child’s pediatrician and possibly a neurologist. The only association migraines have with eyes are some people have migraines triggered by small changes in their eye glass prescription, and some migraines can cause very minor damage to the eyes over time.

Some children and adults are naturally very sensitive to small changes in their eyeglass or contact lens prescription. Every time there is a minor change they start having headaches, and learn quickly it is time for an appointment with the optometrist. Autistic children may have somewhat of an opposite type of  problem, and clearing up vision too much may contribute to sensory overload and headaches.

Rare causes of headaches around the eyes are tumors around the optic nerve or eye. Inflammation inside the eye that sometimes accompanies childhood arthritis and other autoimmune diseases can cause eye pain, headaches, and light sensitivity. Unusual light sensitivity, red eyes, headaches, abnormally small or large pupils, changes in behavior and school grades can be indicators of illicit drug usage, now common even in elementary schools. School counselors usually have a pretty good idea who is involved and it is a quick call.

A sinus infection can result in headaches around the eyes and in rare cases even sight loss.

Never forget lack of sleep. Fatigue, poor diet, caffeine crashes, dehydration, and stress probably cause most headaches. Preventative eye exams for children always helps rule out some easily fixed causes so don’t neglect an eye check up if your child is being afflicted with headaches.

Last, but not least is the friend who just got new glasses syndrome. All of a sudden your child develops blurry vision and headaches. Fortunately, an optometrist can diagnose this case pretty easy. Sometimes you may want to consider discussing in advance with your child’s eye doctor a placebo pair of glasses  with little or no prescription. Occasionally with a strong willed child it is easier to just let them wear a pair for six months  and spend your energy on the bigger battles.

Ten Reasons To Be More Concerned About Your Child’s Headaches

1. Headaches that are continuing to get worse or becoming more frequent

2. Headaches that occur in the same area of the eye or head

3. Headaches that wake children up at night or are present at the beginning of the day

4. Headaches accompanied by double vision, blurred vision, or abnormal eye movements

5. Headaches that cause a child to cease their normal activities

6. Any headache that occurs after a fall, possible bump to the head, or after loss of consciousness or

memory, however brief

7. Any headache that is accompanied by changes in responsiveness, sluggish thinking, slurred

speech, changes in balance, different pupil sizes, severe vomiting, droopy half open eyes, altered or

unusual behavior

8. Headaches that only occur when a specific person is around (possible abuse)

9. Headaches occurring in other people present (possible carbon monoxide poisoning)

10. Severe Headaches with very rapid onset, high fever, stiff neck

If you have concerns about your child having mild or moderate headaches, starting with appointments with your optometrist and family doctor makes sense, proceeding to a neurological evaluation if needed.

Dry Eye Epidemic

Woman With Dry Eye SyndromeThere is a new epidemic that is running rampant across the United States. Fortunately it is not contagious and not life threatening like the Swine Flu, but there is a tremendous cost in terms of loss of quality of life and vision. This epidemic is often referred to as Dry Eye Syndrome and effects millions of people with symptoms ranging from mild, vague discomfort to completely disabling pain and chronic blurred eyesight.The Dry Rye Epidemic has multiple causes.

  • Time spent by adults on computers and youth on video games dramatically lowers the normal blink rate and causes the eyes to dry out faster than normal.
  • Historically, before mass consumption of processed foods, for every unit of omega-3 fat we consumed in our diet we ate 1 to 4 units of omega 6. Today that ratio has shifted to 20-30 times more omega 6 fats in our diets than omega-3. This has caused an outbreak of disease of the eyelid margins and resultant poor tear film quality.
  • As the population has aged there has been an increase due to normal, age related decreases in tear production.
  • The large increase in prescription medication use has resulted in numerous drugs being used that contribute to dry eyes. Medications for blood pressure, depression, allergies, pain, and many other conditions can have drying effects.
  • Millions of women treated with hormonal replacement therapy in the past may have increased the dry eye epidemic.
  • With the shift to industrialized society, working indoors the patterns of airflow and humidity have been altered and effect the incidence of dry eyes.
  • The long term shift away from water consumption towards caffeinated beverages and colas aggravates dry eyes in some individuals.
  • Even the increase in obesity leads to low grade inflammation in the body which can decrease tear production.
  • Many eyedrops, prescription and over the counter, can  cause or aggravate dry eyes to the point that some patients discontinue their treatment and suffer sight loss.
  • Perhaps the worse culprit in the dry eye epidemic is LASIK eye surgery. The nerves that drive the eyes to blink are cut by the surgery and dryness is a frequent complaint after surgery. The dryness sometimes persists and requires long term treatment.

Fortunately we have many options today, including prescription eye  medications, new contact lens materials, glaucoma  eye drops designed for dry eyes, nutritional supplements, and even  micro-plugs implants for the tear ducts to help increase the basal level of moisture. Have You Ever Worried About Not Passing Your Eye Exam?

Testing your eyes for prescription glasses or contact lenses seems like a guessing game but it really is more accurate than you think. The first part of the exam is usually an autorefraction. Autorefractors are computerized instruments that analyze your eyes “refractive state” or how much of a prescription you need. Autorefractors are considered to be objective tests since that have no reliance on responses from the patient. Utilizing a LASER beam and complex “star wars” mathematics they only require a few seconds to analyze the prescription for your eyes. 

 

Which Is Better, Number One Or Two?

Most people don’t like either choice and feel like they are choosing something that is blurry. To some extent that is true and a normal part of the way eye testing has been done for years. Vision tests are done by a “forced choice” method. That really means we make you choose between two views, even when there is little or no difference at times. This may seem counter intuitive, but it does help find a more accurate reading for your eye glass prescription. If you are old enough to remember radios and TVs that had knobs to tune in the stations you will remember bracketing or tuning above and below the station in smaller and smaller steps to fine tune the end point. Subjective eye testing works the same way. Forced choice is well known in psychological testing, and vision testing really is a neuro psychological form of testing.

But Its Getting Worse Not Better!

Most of the time there is a “better view” between one and two. Sometimes there isn’t. At the endpoint of testing there is no longer a JND or just noticeable difference between the two views. The human perceoptual system can only differentiate down to a degree that is limited by the cell sizes in the retina of the eyes. After this point your responses to subjective testing are no longer reliable. The loss of reliability is in an  ironic way an assurance of reliability. If you have been accurately closing in on an endpoint or tuning spot, and then start becoming random in your responses, chances are good the prescription will be fine tuned and accurate. On the other hand, if you are not accurate in narrowing the range at first, or do not display randomness at the end over a small area, the subjective vision testing may not be so good.

 

All Is Not Lost!

No Worries! If that is the case we simply revert to relying more on objective tests independent of your subjective “better of worse” answers. In addition to the autorefractor findings, we will look at your old prescription and how well you see with it. Combined with your visual acuity when you are not wearing glasses we have a fairly good idea of what strength prescription eyewear you need. There is an older technique referred to as Retinoscopy that adjusts a beam of light with lenses to find your prescription. This was the precurser to today’s computerized autorefractors. It also requires no verbal response. The advantage of retinoscopy is the ability of the eye doctor to monitor any variations in the distance you are focusing at. It adds a third opinion into the decision making when needed. 

I Dont Want To Be Stuck With The Wrong Prescription! (The Last Front On The War Against Indecision)

When all else fails, we will examine your eyes after dilation drops have kicked in. They also have the wonderful side effect of disrupting focusing up close. That allows your eyes to be checked when they can no longer change focal distance and add errors into the readings. Autofraction after dilation can add yet another way of testing your eyes. Drops will alter the prescription slightly, but this change is predictable and can be compensated for. Sometimes we will take this reading a few times and even redo retinoscopy after dilation. Usually we can correct your vision to 20/20 even if you can’t make up your mind between one and two! People often wonder how young childrens eyes can be measured. The  beauty of objective eye tests are that we can completely circumvent subjective responses with children.

So Why Not Let The Eye Computer Do All The Testing?

The short answer is it is not quite as good as subjective testing. The other answer is what would I do go fishing? Not really, since the “refraction” is only a small part of testing eyes to assure vision is working correctly and the eyes are staying healthy. Someday computers will probably replace subjective tests entirely but that appears to still be a long way off. Autorefractors have been a blessing to underdeveloped countries where healthcare resources are severely limited. Technology continues to progress with new and refined ways of objectively measuring eye sight. Measurements are just starting to enter the realm where they can exceed your capacity to discriminate. Theoretically that is great but if you really can’t tell the difference from a visual perceptual level, it may not be a true improvement. They are also starting to exceed our ability to make prescriptions, but that part will catch up rapidly. The saying used to be 20/10 in 2010 (implying people would be able to wear lenses that improved their vision form 20/20 to 20/10 by 2010. We kind of missed that milestone but someday!!! Of course the years are going the wrong way now, lets just hope its not 20/30 in 2020!
